SCDHHS ICD-10 Contact Form

The United States health care system is currently scheduled to transition from ICD-9 to ICD-10 set of diagnosis and inpatient procedure codes in 2015.  As mandated, the South Carolina Department of Health and Human Services (SCDHHS) will be implementing the new ICD-10 codes.

In the coming months, you will be receiving many communications from SCDHHS that may provide you with our progress, address any concerns or risks that have been brought to our attention, and/or seek additional information from you, our customers.  As part of that process, SCDHHS needs to identify the PRIMARY and SECONDARY contact persons that are responsible for disseminating information to your organization.

Please take a few moments to provide us with this information to ensure that we are communicating with the right individuals as we set off on this exciting journey together.

IMPORTANT NOTE!  The information collected in this survey is secure and confidential.  It will nto be used for anything other than ICD-10 implementation.
